New Patient Forms For Counseling & Therapy Sessions

Due to the COVID-19 Pandemic

We are providing Telehealth & phone call sessions. We have changed how our office does things to accommodate the need for social distancing  & limit person to person contact as much as possible. For a smooth experience:

• New Patient Packets must be received by the office prior to being scheduled. You can drop it off at the office, mail it or fax your packet over. We also need copies front and back of valid identification as well as insurance cards. So we can verify your insurance.
• When the office has received your packet we can then get you scheduled with one of our counselors.
• After your initial session, you can call the office to pay any co-pay that you may have & to schedule your next session.
